The Student Health Center is located at Linderman Education Center at 124 3rd Ave East in Kalispell, and Glacier HIgh School.
We proudly provide affordable medical, dental, and behavioral health services that are compassionate, integrated and accessible to all Kalispell School District students. 

Greater Valley Health Center is a non-profit, Federally Qualified Health Center (FQHC) committed to serving our community with medical, dental, behavioral health, and substance use treatment services.  Our mission is to provide the greater Flathead Valley with exceptional, patient-centered care regardless of ability to pay.

Medical Services Available at Linderman Education Center Tuesdays and Thursdays 1-4:30 p.m. and at Glacier High School Wednesdays 8:00 am - 12:00 pm. 
* Please Note: Starting June 12th through the summer break, the Student Health Center will be only at Glacier High School. Tuesdays 8:00am - 11:30am & Thursdays 1:00pm - 4:30pm

Psychiatric Services Available the third Tuesday of the month 9 a.m. – noon

Behavioral Health Services Available Monday – Friday 8 a.m. to 4 p.m.

Enrollment (click on the blue links below for enrollment forms)

Become a patient of the Student Health Care Clinic at KPS by completing the enrollment packet, which includes a Registration Form, a Consent for Service, and a Release of Information form.

Patient Registration Form – Enrolls you as a patient of Greater Valley Health Center.   The form includes questions required by the government as a FQHC.  Once you complete the form, your record will be maintained in Greater Valley Health Center’s electronic medical record.  Those records are available only to your medical provider and not to school personnel.

Consent for Service – Indicates your consent for service for yourself (adults) or for your child.  The consent provides guidance to the services that staff can provide if you come to the clinic.  Only the services on the consent will be provided unless staff contact you directly and obtain verbal consent.

Release of Information Form – Allows services staff to release your medical record information to another provider.  If you have a primary care provider, signing this form will allow services staff to send a record of your visit to your primary care provider so that he or she is aware of the care you received.

Billing

Greater Valley Health Center will bill Healthy Montana Kids, Medicaid, Medicare, and most insurance companies.  Copayments and deductibles will be billed to the patient’s guarantor.  We accept ALL patients regardless of ability to pay.  Please present your insurance card at check-in. We can also schedule appointments for assistance with insurance enrollment at our main clinic in central Kalispell.

Prescription Services

Providers are able to write prescriptions for over the counter and prescription medications. 

Many medications are available at low prices through Greater Valley Health Center’s Discounted Pharmaceutical Program with Sykes Pharmacies in Kalispell and Columbia Falls.
